2020/10/01 Influenza Vaccination Guide for Fiscal 2020

As in 2020, PwC Health Insurance Society (“Kempo”) will again subsidize “influenza vaccinations”.

These options are voluntary. Please read the information below and take the necessary steps if you wish to receive a vaccination.

No.	Vaccination Methods	Those Eligible
1.	Vaccinations at Ote-center office （From Oct. 26, 2020 to Oct. 30, 2020）Reservation is REQUIRED. Reserve by 23:59 the day before vaccination.　pre-examination slip Apply from "Reservation system" > "Vaccination Against Flu at Ote Center".	Insured persons only
2.	Vaccination at a Designated Medical Institution (no need to pay at the hospital until Feb. 15, 2021） For children who are eligible for double vaccination, please use "3. Fee adjustments for vaccinations received at other medical facilities".	Insured persons/ Dependents
3.	Fee adjustments for vaccinations received at other medical facilities <<Application acceptance is closed>> ・ Please complete the vaccination and application by February 15, 2021. ・ If there are no deficiencies, the payment will be made at the end of the month following the application month. ・ We will not notify you of the transfer. Please check your account yourself.	Insured persons/ Dependents

Please note:
• To be eligible for subsidy, vaccinations must be carried out between September 1, 2020 and February 15, 2021.
Only available for vaccines approved by the Ministry of Health, Labour and Welfare
• Vaccinations carried out on or after February 16, 2021 are not eligible for subsidy.
• For children who are to be vaccinated twice, please use method 3, ”Fee adjustments for vaccinations received at other medical facilities”.
