今般の新型コロナウイルス感染症予防対策の一環として、２０２０年３月受診に限り
契約医療機関以外受診の場合は、健康診断受診費用を健保組合にて負担し、定期健診を受診
したものとして取り扱います。
以下の方法により申請してください。所定の限度額を上限に受診費用（実費）を支給します。

すでに２０１９年度の健康診断を受診済みの場合、年度２度目の受診はできません。
また、このお知らせは医療機関への立ち入りを懸念する方へ受診を強制するものでは
ありません。受診するか否かは各自の責任においてご判断ください。

【契約医療機関以外の健診費用申請方法】
１．最寄りの医療機関で健康診断を予約・受診
２．健保組合へ以下４点を郵送
　　・健診立替費用請求書
　　・理由書ならび同意書
　　・領収書（原本）
　　・健診結果票コピー

注１）２０２０年３月１日から２０２０年３月３１日受診分に限る

注５）３月受診のため、受診奨励５，０００ポイントの付与なし（例年どおり）

Dear insured persons,

As a part of the new measures to prevent COVID-19 infection, the health checkup expenses at non-contracted medical institutions should be borne by the Health Insurance Society, and that medical checkup shall be treated as a regular medical checkup for the fiscal 2019 by the Health Insurance Society in the case where the checkup is conducted within March 2020.
Please apply by the following [Application Method].
The cost of medical examination (actual cost) shall be reimbursed under the limit of the maximum coverage.

If you have already taken a health checkup in fiscal 2019 (from Apr. 2019 to Mar. 2020), you cannot do that by this method for the second time at the Health Insurance Society's expense.
Also, this notice does not force people who are concerned about entering a medical institution to visit.
Please decide whether to visit or not at your own responsibility.

[Application Method]
1. Make your own appointment and take a medical examination at a preferred medical institution.
2. Mail the following 4 items to the PwC Health Insurance Society, 5-21-14, Sendagaya, Shibuya-ku, Tokyo, 151-0014.
・ The bill for the reimbursement of the health examination
・ The statement of reasons and the written consent
・ The receipt (original)
・ The copy of the health checkup result.

1) This reimbursement is limited to health checkup conducted from March 1, 2020 to March 31, 2020.

4) You can add optional examinations within the scope of the upper limit.
5) You can't get 5,000 points by taking a health checkup in March (This is the same as our usual rule).
