相応の事情により定期健診を受けられない方(*1)は、その事情を添えてこちらよりお申し出ください。(*2)
ただし、その事情の内容によっては「年度を通じて受診をしない理由としては認められない」と判断される場合があります。あらかじめご承知おきください。

注：

(*1) 下記①・②に該当される方は、お申し出不要です。

　①法人・会社にて以下の発令があった方

（海外赴任（本人と家族）／産前産後休職／育児休職）

　②健保の給付実績があった方

（出産手当金／出産育児一時金／家族出産育児一時金／傷病手当金（6か月以上の長期にわたるもの）

(*2) 件名の変更はご遠慮いただきますようお願いします。変更されますと、健保組合での確認ができない可能性があります。

To notify us of circumstances in which you and/or your dependent family member cannot undergo the annual health checkup(*1) , please contact us along with the circumstances. (*2)
However, depending on the circumstances, it may be determined that "the reason for not having health checkups throughout the year is not allowed". Please be aware in advance.

(*1) Those who fall under categories ① and ② below do not need to make a request.
①Those whose PwC issued with the following orders: (Overseas assignment (person and family) / Pre- and post-natal leave / Childcare leave)
②Those who have received health insurance benefits: (Maternity Allowance/Lump-sum Childbirth and Childcare Allowance/Dependent Childbirth and Childcare Lump-sum Allowance/Injury and Sickness Allowance (long-term))
(*2) Please refrain from changing the subject line. If it is changed, there is a possibility that the health insurance society may not be able to confirm it.
