インフルエンザ予防接種費用補助申請書の提出期限は2018年2月15日（木）健保必着です。
指定医療機関での接種も2月15日が最終日です。
Application for flu vaccination fee adjustments MUST be received at Kenpo by Wednesday, February 15, 2018.
